Paraduodenal hernias are congenital internal hernias occur due to malrotation of the midgut, the mesentery fails to fuse with the parietal peritoneum creating a hernia orifice. Here, we report a case of intestinal obstruction secondary to left paraduodenal hernia in a 17-year-old male who presented with diffuse abdominal pain mainly at epigastric region and vomiting. CT abdomen revealed paraduodenal hernia. Management was carried out by exploratory laparotomy, reduction of herniated structures and closure of hernia orifice. Early diagnosis and immediate intervention are substantial to prevent predictable complication and decrease morbidity and mortality. Therefore, paraduodenal hernia should be included in the differential diagnosis of small bowel obstruction.
